1. Documents for proof of identity

  • Voter Identity Card
  • Driving License 
  • Passport
  • Arm’s License
  • Central Government Health Scheme Card or Ex-servicemen
  • Contributory Heath Scheme photo card
  • Ration Card having photograph of applicant
  • Photo identity card issued by the Central Government or a State
  • Government or a Public Sector Undertaking
  • Pensioner Card having photograph of the applicants
  • Elector’s photo identity card

For above all documents only Photocopy is Required

  1. Certificate of identity in original signed by Member of Parliament or Member of Legislative Assembly or Municipal Councilor or a Gazetted Officer
  2. Bank certificate in original on letter head from the branch (along with name and stamp of the issuing officer) containing duly attested photograph and bank account number of the applicant

There 2 Document’s original Copy is Need to Submit 

2. Documents for proof of address

  • Elector’s Photo Identity Card
  • Driving License
  • Passport 
  • Bank Account Statement/Passbook
  • Credit Card Statement 
  • Landline telephone or broadband connection bill
  • Passport of the Spouse
  • Post office pass book having address of the applicant
  • Electricity Bill
  • Depository Account Statement 
  • Latest Property Tax Assessment Order
  • Domicile certificate issued by the Government
  • Allotment letter of accommodation issued by the Central Government or State Government of not more than three years old
  • Property registration document
  • Consumer Gas Connection Card or book or piped gas bill
  • Water Bill

For above all documents only Photocopy is Required

  • Certificate of identity in original signed by Member of Parliament or Member of Legislative Assembly or Municipal Councilor or a Gazetted Officer
  • Employer certificate in Original 

There 2 Document’s original Copy is Need to Submit 

3. Documents for proof of date birth

  • Birth Certificate issued by the Municipal Authority
  • Pension Payment Order
  • Marriage certificate issued by Registrar of Marriages
  • Matriculation certificate
  • Domicile certificate issued by Government
  • Affidavit sworn before a magistrate stating the date of birth by the Government
  • Driving License
  • Passport

4. Documents for proof of office address

  • Credit Card Statement carrying Office Address
  • Bank Account Statement carrying Office Address
  • Employer Certificate carrying Office Address
  • Electricity Bill carrying Office Address
  • Telephone Bill carrying Office Address
  • Depository Account Statement carrying Office Address
  • Office Property Tax Assessment Order bill
  • Bank Account Statement carrying Office Address

For above all Documents only Photocopy is Required

After collecting all those documents,  visit the official website (https://tin.tin.nsdl.com/pan/) and fill the form for “Reprint of PAN Card”. Applicant is required to fill all the columns in the form but should not tick any of the boxes on the left margin. Check below Images and proceed.
